Waiver of Liability: I, the parent/guardian/player, release, discharge and/or otherwise indemnify Paradise Football Club, its affiliated sponsors, employees and associated personnel, including the owners of fields and facilities utilized against any claim by or on behalf of the registrant as a result of their participation. *

General Consent for Medical Treatment: I give my consent to have an athletic trainer, coach paramedic, and/or Doctor of Medicine or dentistry provide medical assistance and/or treatment. I agree to be financially responsible for the reasonable cost of such assistance and/or treatment. *

Code of Conduct & Registration Agreement: I agree to follow the Paradise FC Code of Conduct, which includes respectful behavior towards coaches, officials, and other players; regular attendance at training and matches; no use of drugs or alcohol; no fighting or violent conduct; commitment to sportsmanship and fair play; compliance with League regulations and BFA policies; consent to image and media rights usage for promotional and transfer-related purposes; acknowledgment of medical expense responsibility; and understanding that participation is at my own risk. *

Registration Fees: Registration: $50 | BFA Insurance: $65 (annual, due every January) | Term Fee: $90 (per term, 3 terms/year)
