Complete your online registration to join Paradise FC. All sections must be filled accurately.
Go to FormRequired for all youth players (U9 through U19). Must be signed by a parent or legal guardian.

Download PDFRequired BFA insurance form for all players. Must be completed to participate in league matches and tournaments.
Download PDFSagicor General Sports Group Personal Accident Claim/Injury Form. Use this form to report injuries sustained during club activities for insurance processing.
